Purpose: This document outlines the Standard Operating Procedure (SOP) for all health facilities in the state to submit their Monthly Quality Improvement Report to the Mizoram QA Dashboard. Reports must be submitted on or before the 10th of each month, following the enclosed updated process mapping. Facility- or district-wise data submitted can be extracted from the Dashboard for record-keeping, further analysis, and presentation.
